To complete the necessary paperwork we request you arrive 5 minutes prior to your scheduled appointment time.
The following will be required:
- A current referral from you GP
- Medicare card
- Membership details of your private health fund insurance or workcover insurer
- Any relevant Xray’s, ultrasounds, MRI’s, bone or CT scans
We encourage you to ask questions during your consultation to ensure you understand any treatment or required surgery.
Should you be unable to attend an appointment please contact the practice as soon as possible.
